COVID-19 Effects on Cancer Patients Care Questionnaire

Thank you so much for your interest in our survey. With your participation, we hope to better understand how COVID-19 has impacted cancer patient's care. This is a very complex and ongoing situation that many public health entities are trying to better understand. However, with your help, we hope to find barriers that the pandemic created, possible health issues/delays it caused, how your care could be bettered, and more.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn about your experience.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
